Cost per dose is not cost per milligram
Sellers quote a price per vial. Readers convert that to a price per milligram and stop there. That number is only correct if every milligram in the vial ends up in a dose, and it often does not.
The honest figure is price per vial divided by the number of whole doses you can actually draw. A whole dose is one you can measure and administer in full. Whatever is left over after the last whole dose sits in the vial as a partial amount. It was paid for. It was not used.
So the arithmetic has two steps. First, divide the vial contents by the dose to get whole doses and a remainder. Second, divide the vial price by the whole doses. The remainder never enters the denominator, which is why an awkward dose size raises the real cost per dose even when the price per vial is unchanged.
