What the 28 day window is measuring

The window in the table above is a handling figure, not a potency figure. It describes how long a multi-dose vial is conventionally treated as usable once a preservative-containing diluent has been added and the stopper has been pierced. It is the standard applied to multi-dose vials generally, and it is what handling references converge on for a reconstituted vial held cold.

What sets that number is the preservative in bacteriostatic water, not the peptide. Bacteriostatic water contains an agent that suppresses microbial growth rather than eliminating it. Suppression is finite. Each needle entry through the stopper is a chance to introduce something, and the preservative load is drawn down over the life of the vial.

So the window answers a contamination question. It does not answer a chemical stability question, and it does not answer a potency question. This page cannot tell you what a Liraglutide molecule is doing inside the vial on any given day, because that would require data we do not hold.

Why warmth costs so much

The gap between the refrigerated row and the room temperature row is the most important thing in the table. It is not a modest reduction. The room temperature window is a small fraction of the cold one, and the reason is that preservative effectiveness falls away far faster warm.

Write the reconstitution date on the vial itself, not on a box. The vial is the object that will still be in the fridge in three weeks when you have forgotten. The dry powder row is the useful counterpart: powder is the stable form, good for months to years refrigerated. If you are not going to use a vial soon, the correct place for it is unmixed, with the vendor's date checked.

What the mixing volume does and does not change

Liraglutide is sold in 5 mg and 10 mg vials, and listings show reconstitution in 1 mL, 2 mL or 3 mL of bacteriostatic water. Dose amounts appearing in those same listings run 0.6 mg, 1.2 mg, 1.8 mg and 3 mg.

Changing the water volume changes the concentration, and therefore the number of syringe units that corresponds to a given amount. It does not change the handling window. A vial mixed in 1 mL and a vial mixed in 3 mL sit on the same 28 day refrigerated clock, because that clock is set by the preservative and the stopper, not by how dilute the solution is.

What volume does affect is how long the vial physically lasts you. A vial that empties before the window closes and a vial that still holds liquid on day 28 are different situations.
