What a unit on a U-100 syringe measures

A U-100 insulin syringe is graduated in insulin units, not in milligrams of peptide. One unit is one hundredth of a millilitre. The barrel measures volume and nothing else.

So every conversion here has two steps. First, concentration: milligrams in the vial divided by millilitres of bacteriostatic water added. Second, volume: your dose in milligrams divided by that concentration, then multiplied by 100 to read it as units.

The consequence is that no dose has a fixed unit value. The same 1 mg of Humanin sits anywhere on the barrel depending on how much water went into the vial. A unit number quoted without the vial size and the water volume is meaningless, which is why the table carries all three columns rather than just the last one.

Three things to notice in the chart

First, water volume scales the units linearly while the dose stays the same. A 1 mg dose from a 10 mg vial moves from 10 units to 20 units to 30 units as the water goes from 1 mL to 2 mL to 3 mL. The amount of Humanin leaving the vial is identical in all three rows. More water does not make a dose weaker, it spreads it across more of the barrel.

Second, one row runs off the end of the syringe. A 5 mg dose from a 10 mg vial in 3 mL comes to 150 units, and a standard U-100 barrel holds 100. That row is flagged because it cannot be drawn in one go. It would need two draws, which doubles the chance of a measurement error, or a different reconstitution volume.

Third, the 3 mL rows give a concentration of 3.33 mg/mL, a repeating decimal rounded for display. The unit figures there are clean whole numbers only because these particular doses divide neatly. Change the dose and you will land between graduation marks.

Humanin is sold in 5 mg and 10 mg vials. The chart here is computed for the 10 mg vial only. A 5 mg vial at the same water volume gives a lower concentration, so any given dose occupies more units. Do not read those figures across from the 10 mg rows.
