What the conversion actually is

An insulin syringe is not marked in milligrams. It is marked in units of volume. A U-100 barrel is graduated to 100 units across 1 mL, so one unit is one hundredth of a millilitre. The syringe has no idea what is dissolved in the liquid it is drawing.

That means the number of units you draw is set by two things: the milligram dose you want, and the concentration of the solution in the vial. Concentration comes from dividing the vial contents by the volume of bacteriostatic water added. A 20 mg vial in 1 mL is 20 mg/mL. The same vial in 2 mL is 10 mg/mL. The powder did not change. Only the dilution did.

Once you have a concentration, the dose in milligrams divided by that concentration gives millilitres, and millilitres multiplied by one hundred gives U-100 units. The table on this page runs that calculation for the vial and water combinations most often paired with Epitalon. It converts a dose you already have into a mark on a barrel. It does not tell you which dose to pick.

Reading the chart: three things to notice

First, adding more water does not weaken the dose. It changes how much liquid carries that dose. A 5 mg dose is 25 units at 1 mL of water and 75 units at 3 mL of water. Both deliver 5 mg. The larger number is more liquid, not more compound. This is the single most common misreading of a reconstitution chart.

Second, the two variables move independently and both matter. Doubling the dose from 5 mg to 10 mg at a fixed 1 mL doubles the units from 25 to 50. Doubling the water from 1 mL to 2 mL at a fixed 5 mg also doubles the units, from 25 to 50. Identical marks on the syringe, different amounts of compound. A unit figure is meaningless unless the vial size and the water volume travel with it.

Third, more water buys you resolution. At 20 mg/mL, a 5 mg dose sits at 25 units and small errors in drawing translate into larger milligram errors. At 6.67 mg/mL, the same dose spreads across 75 units, so each unit represents less compound and the mark is easier to hit accurately.

When the dose exceeds one barrel

One row in the table crosses a physical limit. A 10 mg dose from a 20 mg vial reconstituted with 3 mL comes to 150 units. A U-100 syringe stops at 100. That volume cannot be drawn in a single fill.

This is not a warning about the dose. It is a note about equipment. The arithmetic is correct and the concentration is fine, but the combination requires either a larger syringe or more than one draw, which adds handling steps and room for error.

Water volume is therefore best chosen with the intended dose in mind. Heavy dilution spreads small doses across more of the barrel. For larger doses out of the same vial, it pushes the required volume past what the syringe can hold.

The method does not change for the others. Divide the vial contents by the water added to get milligrams per millilitre, then divide your dose by that figure and multiply by one hundred. A vial holding more powder reconstituted with the same water gives a stronger solution, so the same dose lands on a lower unit mark. A smaller vial in the same water gives a weaker solution and a higher unit mark for the same dose. A unit figure copied from one vial size and applied to another will be wrong.

How many units is 5 mg of Epitalon?
It depends entirely on how the vial was reconstituted. From a 20 mg vial in 1 mL of bacteriostatic water, 5 mg is 25 units on a U-100 syringe. From the same vial in 2 mL it is 50 units, and in 3 mL it is 75 units. The unit figure is meaningless without the vial size and water volume attached.
Does adding more bacteriostatic water make the dose weaker?
No. The vial contains a fixed amount of powder regardless of how much water you add. More water spreads that powder through more liquid, so the same milligram dose occupies more volume and reads as more units on the syringe. The compound delivered is unchanged.
How much water should I use to reconstitute an Epitalon vial?
There is no single correct volume, and this site does not recommend one. The chart shows 1 mL, 2 mL and 3 mL because those are the volumes commonly used. More water gives finer resolution on the syringe for small doses, but pushes larger doses toward or past the 100 unit limit of a U-100 barrel.
What do I do if the calculated units exceed 100?
A U-100 syringe holds 100 units. The 10 mg dose from a 20 mg vial in 3 mL works out to 150 units, which will not fit in one draw. That combination needs a larger syringe or a more concentrated mix. Reconstituting with less water lowers the unit figure for the same dose.
Can I use this chart for a 10 mg or 50 mg Epitalon vial?
Not directly. The table is computed for the 20 mg vial. A different vial size in the same water volume produces a different concentration, so the unit figures shift. Recalculate using the vial contents divided by the water added.
How often should Epitalon be taken?
